Cancellations List

• Fill out the form below to be added to our cancellations list for the next 30 days.
• We will contact you in the coming days/weeks with any openings that correspond to the dates and times you've requested.
• If contacted, please confirm that you can accept the appointment within 1 hour.  Please respond even if you cannot take the appointment.  This is a big help to us in managing our cancellations list. 
  
• After 1 hour or if you have declined the appointment, it will be offered to the next client on our cancellations list.
  
• Cancellation requests are kept for 30 days.  After 30 days, please feel free to fill out a new request to be added to the list again.
